Privacy Notice

The Department of Child Safety, Seniors and Disability Services is collecting your personal information on this form in order to assess your eligibility for the Seniors Card, Seniors Card+go or Seniors Business Discount Card. Your personal information will be used by authorised officers within Smart Service Queensland, Department of Transport and Mains Roads Queensland and contracted service providers to administer and evaluate the Seniors Card and Seniors Business Discount Card scheme and may be disclosed to other government agencies. Your personal information will be managed in accordance with the Information Privacy Act 2009.

Permanent resident of Queensland

To be a 'permanent resident of Queensland', you must live in Queensland for at least 6 months a year. Your home address must be shown on your driver licence or be registered in your name with (at least) 1 of the following:

  • Electoral Commission of Queensland
  • Centrelink
  • Commonwealth Department of Veterans' Affairs
