Photo identification card sample

The photo identification card:

  • provides proof of age for Queenslanders aged 15 years or older
  • is an ideal form of photo identification for people who are not old enough to get a driver licence or don’t hold a driver licence or passport.

The photo identification card was introduced in April 2019 and replaced the adult proof of age card which is no longer issued.

Adult proof of age cards issued prior to April 2019 are still valid and should be recognised until the card expiry date shown on the front of the card.

Adult proof of age cards replaced the 18+ card. If you hold an 18+ card, it is still valid.

If you have a photo identification card, you can download and use the Digital Licence app. You can use the app to store your information and share details of your card, any time you need to provide your information or prove your identity. If you have an adult proof of age card, you can also use the app – your card information will appear in the app as a photo identification card.

Apply for a photo identification card—in person

You can apply in person for a photo identification card at a transport and motoring customer service centre, participating Queensland Government Agency Program Office (QGAP), or a police station that provides licensing services.

How to apply—in person

To apply for the card you must:

You may need to have your photo taken when you apply for a photo identification card. You will not receive your card straight away, it will be sent you by mail by standard post.

When you apply for your photo identification card you will be asked if you want your residential address to be shown on the back of your card.

Photo identification card and adult proof of age card holders can add or remove an address on their card online or in person.

We will post your photo identification card by standard mail to your mailing address within 21 days. If you have signed up for e-reminders we will email you when your photo identification card is posted.

Apply or replace a photo identification card—living in a remote area

You may be eligible to apply for a photo identification card by mail if you live in a remote area of Queensland and can't get to a transport and motoring customer service centre, participating Queensland Government Agency Program Office (QGAP), or police station that provides licensing services.

If your photo identification card has been lost, stolen, damaged or destroyed you may also apply for a replacement by mail.

How to apply if you live in a rural or remote area

Step 2: Complete the form

Complete all sections of the photo identification card application / replacement application (F4772)

You must sign this form in the presence of one of the approved witnesses:

  • medical practitioner
  • police officer
  • lawyer or judge
  • Justice of the Peace or a Commissioner for Declarations
  • notary public or a person authorised by law to witness and sign declarations
  • consular or ambassadorial officer.

Make sure you have your photos with you so that they can be signed by the witness at the same time (see step 4).

Step 3: Make sure to sign the form

You need to print this form on A4 white paper to make sure it is the correct size for digital imaging of your signature.

Make sure you use black pen only to complete all sections of the form and sign your name in both specimen signature boxes.

Step 4: Photos

You must provide 2 identical colour photos. You will need to get an approved witness (see step 2 for who is an approved witness) to sign and endorse the back of both photos.

The words that must be used are:

"I certify this is a true photograph of [applicant’s name] in my presence"

The photos must:

  • be 35mm wide by 45mm long in size and must not be more than 6 months old
  • be passport quality (automatic machine photographs such as those taken from a photo booth, are not acceptable)
  • be printed on high-quality paper, at a resolution of 600dpi or higher
  • not be digitally altered or manipulated, for example, photo editing to remove spots.

Glasses or sunglasses must be removed for the photograph, even if they are normally worn.

Step 5: Evidence of identity

You will need to provide evidence of your identity.

If you have changed your name, or the details of your name are different on your identification documents, you will need to supply evidence of your name change. The photocopies of your evidence of identity documents must be signed and endorsed by an approved witness (see step 2 for who is an approved witness) with the following words:

"I have sighted the original document and certify this to be a true copy of the original"

Step 6: Pay the fee

You will need to pay the fee for your photo identification card by cheque or money order. Do not send cash.

Alternatively, the fee may be paid directly into your account at a transport and motoring customer service centre, participating Queensland Government Agency Program Office (QGAP), or police station that provides licensing services.

If you choose to have someone pay the fee directly into your account at a transport and motoring customer service centre, the fee must be paid before you send the completed application to us.

Step 7: Submit your application

There is a checklist in the photo identification card kit. When you have prepared your application, go through the checklist and make sure you have included everything to prevent delays with processing your application.
